No, the card slots are the same size but the North American SMS has a different BIOS than the Japanese SMS (its BIOS is a later version). The only other difference is what Sega chose to call them. Sega My Cards in Japan. Sega Cards everwhere else.

I don't have any European cards to try, because the U.S. got the most card games outside of Japan. Japan got the most of course.

If you want a lot of bang for the buck (and you can find one that is affordable) I can reccomend the Japanese SMS. It is compatible with the SG-1000, Mk. III, SMS, and Othello Multivision. It has built-in the Yamaha FM Sound chip (in addition to the Texas Instruments Sound chip), plus bult-in 3D hardware and Rapid Fire. No built-in game however. It has become semi-rare, so is usually a little pricey these days. Even harder to find is an Othello Multivision (2 models, FG-1000; FG-2000), a liscensed SG-1000/SG3000 clone manufactured by Tsukuda Original. It is extremely mega-rare!. It DID have a built-in game, Othello. There were game released for the Multivision, such as Sannin Mahjong,Challenge DerbyOkamato Ayako no Match Play GolfSpace Armor, and 007 James Bond. However, the Japanese Master System will play those as well.
